분할된 계획과 완료 로그를 Task id 기준으로 집계해 마일스톤 동기화가 완료 작업을 누락하지 않도록 한다.

fix(agent-ops): 외부 실행 차단을 사용자 리뷰로 전환한다
자동 실행할 수 없는 외부 검증에서 후속 PLAN을 반복하지 않고 사용자 조치와 재개 조건을 파일 기반 gate로 남기기 위해 정책과 런타임 검증을 맞춘다.
